APPLICATION PROCEDURE

Head of household must come in
person to the Good Samaritan Alliance building. -
Bring with you:

The Social Security card for
each person living in the house.

Personal identification
(driver’s license, etc.).

Proof of income (most recent pay
slip, bank statement, Social Security Letter, Food Stamp
letter, etc.).

Be interviewed to determine
eligibility.
NOTE: Assistance is granted
based on eligibility, availability of funds and goods at the
time of the interview.
